We have received your application for registration as a Registered Nurse (RN)/Registered Practical Nurse (RPN) in the Special Assignment Class.

If you haven't already done so, you will need to send the following documents to CNO:

  • A completed Verification of Appointment or Special Assignment Form
  • Proof of your citizenship, residency status, or authorization to practice nursing in Ontario
  • Evidence of language proficiency in English or French
  • You will also need to request a police criminal record check through Sterling, a secure third-party criminal record check provider. CNO does not accept police criminal record checks from any other sources. Your police criminal record check is valid for only six months after it is issued. Please keep this timeline in mind when you start the application process. If your police criminal record check expires while your application is in progress, you will need to request a new one. Please arrange for the appropriate organizations to send the following verification forms to CNO:

All applicants must successfully complete an online jurisprudence examination. Once you have submitted your online application and fee, you will receive information about how to access the secure testing site.

To check the status of your application and to see a list of your requirements, login to Maintain Your Membership, select “View Open Applications” and select your application. A registration requirement will be marked as “outstanding” either because information has not been received, or because the information that has been received is still being reviewed by CNO.

Throughout the application process, we will send you messages in the online portal’s message centre.

Has your contact information changed?

If your mailing address, telephone number or email address has changed, please use the “update my personal information” feature in the online portal. However, if your name has changed, you must send a letter to CNO along with your official name-change document, such as a marriage certificate. In the letter, please indicate how you would like your new name to appear.
