1 Executive Summary

The New Registrations Report 2022 presents annual statistics about nurses who became newly registered between 2015 and 2022. The report focuses on three nurse types: Registered Nurses (RNs), Registered Practical Nurses (RPNs), and Nurse Practitioners (NPs). New registrations can be broken down into two types: 1) new registrants, who registered with the College of Nurses of Ontario (CNO) for the first time, and 2) additional registrations, nurses who were already registered with CNO and completed a new registration (for example, an RPN who adds an additional registration to also become an RN) or moved from the General to the Extended class. Trends in the number of new registrants and additional registrations by nurse type, location of education, and age were examined.

The total number of new registrations surpassed 15,000 for the first time in 2022, with 12,385 new registrants who joined CNO for the first time and 3,050 additional registrations gained by nurses who were already registrants. Of the new registrants, RNs and RPNs were almost evenly split (49% vs 51%), while nurses who gained additional registration were primarily RPNs who completed an RN registration and made up 86% of all additional registrations.

While the majority of new registrants are still graduates from Ontario programs, the proportion educated abroad reached 41% in 2022. A total of 5,125 newly registered internationally educated nurses (IENs) joined CNO this year for the first time, more than double those in 2021 and triple the number in 2020. The number of additional registrations from nurses with international education also more than doubled in 2022, increasing from 785 to 1,834.

Overall, CNO registered more new registrants and processed more additional registrations in 2022 than ever before. The increases were driven by IEN registrations across both types and are largely attributable to purposeful efforts to increase the supply of nurses.

2 Introduction

CNO is the regulatory body for nurses in Ontario, Canada. Our purpose is to protect the public by promoting safe nursing practice.

One of CNO’s main functions is to register nurses in Ontario by ensuring they meet the requirements to practice safely. Only a person with a valid CNO certificate of registration can practice as a nurse in the province and use the titles Nurse, Registered Nurse (RN), Registered Practical Nurse (RPN) or Nurse Practitioner (NP).1 Nurses may register with CNO at any time throughout the year and anyone who holds a certificate of registration is considered a registrant. Registrants may hold more than one certificate of registration.

There are two categories of registration at CNO: RN and RPN, with different classes for each. Nurses in the General and Extended classes that meet all the requirements can work anywhere in Ontario without restrictions2. The RNs registered in the Extended Class are known as Nurse Practitioners; there is no extended class for RPNs. Those in the Temporary Class have limits on where and how they can work,3 and those in the Non-Practising Class cannot work as nurses.

This report contains annual statistics about nurses who became newly registered in the General and Extended classes, between 2015 and 2022. New registrations can be broken down into two types: 1) new registrants, who registered with CNO for the first time, and 2) additional registrations, nurses who were already registered with CNO and either completed a new registration or moved from the General to Extended class. Historically, this report presented all new registrations together as one group. The 2022 and future versions will examine trends by these types separately as they impact CNO’s total registration numbers in different ways and have distinct registration and demographic profiles. These changes will also be reflected on the Registrant Statistics webpage that shows the number of new registrations each month. Tracking trends in new registrants and additional registrations over time provides valuable information for health human resource planning and sheds light on how nurses move through different categories and classes within CNO.

3 Methods and Notes

Nurses are eligible to register with CNO after they meet all the registration requirements4 for a particular category and class. Their certificate of registration allows them to practice nursing in Ontario as one of three nurse types:

  • RN: Registered Nurse in the General Class
  • RPN: Registered Practical Nurse in the General Class
  • NP: Registered Nurse in the Extended Class

All new RN, RPN, and NP registrations granted in the calendar years (between January 1st to December 31st) from 2015 to 2022 are included in this report. Nurses are categorized as new registrants or additional registrations based on their registration history. Nurses may hold registration in more than one category (RN or RPN) at the same time, but not multiple classes in the same category. For example, a nurse may hold both RN General and RPN General registration, but cannot be in the RN General and Extended Class (NP) simultaneously. For simplicity, throughout this report nurses will be categorized based on their nurse type, not their category and class.

3.1 New registrants

New registrants are nurses who register with CNO for the first time in either the General or Extended Class, regardless of category. They represent new nurses to the system and can only be categorized as a new registrant once in their career. For example, a new graduate who registers with CNO for the first time as an RPN. For any subsequent registration they will not be considered a new registrant. Registrants in the Non-Practicing Class who move back to the General Class and retired nurses who return to practice are considered reinstatements and are not covered in this report.5

3.2 Additional registrations

Additional registrations are nurses who were already registered with CNO when they gained a new registration and either a) completed an additional registration in a new category, or b) moved from the General to the Extended class. They represent enhanced capacity to the system, but not new nurses. For example, an RPN that gains additional RN registration or an RN that becomes an NP. In both cases, because the nurse already held CNO registration, they are considered additional registrations as opposed to new registrants.

There is a small number of applicants every year who register as an RN and RPN on the same day. In these cases, if they have no previous CNO registration, their RN registration is categorized as a new registrant, while their RPN is considered the additional registration.

3.3 Other demographics

Location of nursing education indicates the province or country of a registrant’s education, as reported by the registrant when applying to CNO. This report groups locations into the following categories: Ontario;6 other Canadian provinces or territories; and international.

Age is calculated based on the date when nurses successfully registered with CNO.

Overall, total annual new registrations to CNO increased from 9,310 in 2015 to 15,435 in 2022. Compared with 2021, there was a 24% increase in the number of nurses who registered for the first time, gained additional registration or moved from the General to the Extended Class.

The majority of all new registrations come from new registrants, as 12,385 nurses registered in the General or Extended Class for the first time in 2022 compared to only 3,050 additional registrations from nurses who were already CNO registrants. While the number of additional registrations is significantly lower than those of new registrants, proportionally, the number is growing.

In 2015, additional registrations made up only 9% of all new registrations compared to almost 20% in 2022.

While most new RNs and RPNs are nurses never previously registered with CNO, most new NPs are RNs who moved from the General to the Extended class and are therefore categorized as additional registrations.

The number of nurses who registered for the first time with CNO reached 12,385 in 2022, exceeding the previous annual record set in 2018. Compared to 2021, the number of new registrants rose by 19%.

The share of new registrants that were RNs was 49% in 2022, while RPNs made up 51%, and NPs only 0.3%. Since 2015, RPNs have accounted for over 50% of new registrants each year except for 2019 and 2020, though they make up less than 35% of total registrations.7

Over the last three years, the total number of newly registered internationally educated nurses (IENs) has risen dramatically, more than tripling from 1,609 in 2020 to 5,125 in 2022. Last year saw the largest ever year-over-year change in new registrant IENs, increasing by 109% from the year before, and the highest number of new IENs registered in a single year. As a proportion, they made up 41% of all new registrants, up from 24% in 2021, and almost 60% were RPNs. The growth in 2022 is largely driven by CNO’s modernization of its applicant assessment,8 which enables IEN applicants to meet their nursing registration requirements faster, and by directives from the Ontario government to register more IENs.9 One CNO modernization effort facilitated the registration of over 1,000 IENs, the Supervised Practice Experience Partnership (SPEP) program,10 was launched in January of 2022 to assist international nurses in completing their evidence of practice and language proficiency requirements.

Overall, Ontario graduates still made up the majority of new registrants, though it varied across nurse type. In 2022, they made up 58% of new RNs but only 50% of new RPNs. Historically, Ontario graduates made up 70-80% of new registrants for both RNs and RPNs. Between 6,500 and 7,500 Ontario graduates typically register with CNO for the first time each year, with 2018 being a notable outlying year. In 2022, the 635 new registrants educated in other Canadian provinces made up only a small fraction of all new registrants. Nurse Practitioners registering in Ontario for the first time were the exception and were predominantly educated outside Ontario. They only made up a small number of overall new NP registrations; however, as over 90% are considered additional registrations. The number of new NPs trained outside Ontario and abroad entering as new registrants is minuscule.

Compared with the previous year, 2022 saw an overall 24% increase in new registrants educated in Canada but outside of Ontario. While RNs made up the bulk of these new registrants, RPNs had the largest growth, going from 100 to 154 between 2021 and 2022. Quebec and Alberta are the two provinces where the majority of Canadian-trained new registrants are educated.

Since 2015, nurses educated in India and the Philippines have made up the vast majority of new registrant IENs. This year saw even more, with 3,037 and 1,560 new IENs receiving their education in those two countries, an increase of 129% and 110% respectively from 2021. The total proportion of IENs coming from India and the Philippines has steadily risen over the last eight years, making up roughly 80% of all new IENs in 2015 to now almost 90% in 2022. Nepal, USA, and Nigeria make up the next three countries, with less than 5% between them. The growth in the number of new-registrant IENs from India in 2022 is most pronounced in the RPN group. Almost 2,000 Indian-educated RPNs registered with CNO for the first time, more than the total new IEN RPN registrants in 2021 across all countries. Since 2015, new NPs trained abroad previously only came from the USA.

The average age of new registrants in 2022 is lowest in the RN group at 28.3 years followed by RPNs 30.5 years. New registrant NPs are the oldest at 39.3 years. The average age of new registrants has been increasing in the RN and RPN groups, primarily driven by the increasing number of new IEN registrants.

Similar to the number of new registrants, the number of additional registrations - nurses completing a registration in a new category or moving from the General to the Extended Class - added by CNO registrants was the highest since 2015. In 2022, 3,050 additional registrations were completed, of which 2,168 (86%) were RNs, highlighting that nurses who gain additional registration are mostly RPNs who add an RN registration. New NPs are also predominantly additional registrants, as the majority are CNO registrants that change class. Of the 80 nurses who added an RPN registration in 2022, 74% were added concurrently with RN registrations on the same day.

Relative to 2021, the number of additional registrations increased by 47%. Compared with 2015, the number has more than tripled, driven by a five-fold increase in the number of RN additional registrations, suggesting RPNs are increasingly completing additional education to extend their scope as RNs. In contrast, the number of additional NP registrations has fluctuated between 257 and 384 over the last eight years. Registered Nurses saw the largest increase from the previous year with 61% more additional registrations in 2022.

In 2022, the number of additional international registrations surpassed those from Ontario graduates for the first time. As a proportion, IENs made up 60% of all additional registrations compared with only 38% in the year prior and were almost all RNs. Like the increases seen in new registrants, this is a reflection of CNO’s modernization of its applicant assessment and external directives to register more IENs.11 Of the almost 7,000 new IEN registrations in Ontario in 2022, only 1,834 were additional registrations, suggesting the policies to increase IENs in the provinces led to substantially more new registrant IENs than additional registrations IENs. The number of additional Ontario graduates has also increased over time, but at a steadier rate, growing from 664 in 2015 to 1,145 in 2022.

The total number of additional registrations received from nurses educated in another Canadian province, who already held CNO registration, is small. Of the 71 additional registrations, 56 were educated in Alberta and, of those, 45 were NPs. Of the 45 additional registration NPs from Alberta, 100% were educated at Athabasca University, likely due to remote/online NP programs being offered.

In 2022, India and the Philippines were the top international countries of education for new CNO registrations, making up more than 90% of all international additional registrations. While nurses educated in India have made up the majority of additional registrations over the last eight years, the proportion from the Philippines has climbed from 16% in 2015 to more than 30% in 2022. These increases, again, are driven primarily by RPNs gaining RN registration. It should be noted that international RPNs registered with CNO who complete an Ontario RN baccalaureate program are not included in this table because their additional registration is considered Ontario educated. Moreover, on their 2022 annual renewal, 67% of nurses who gained additional registration using international education indicated they were practicing in Ontario and 71% have had concurrent RN and RPN applications open at one point. This suggests these nurses often apply to both the RN and RPN general classes when they seek CNO registration but do not initially meet the RN requirements. The programs and policy changes implemented in 2022 appear to have supported this sub-group of nurses in achieving their RN registration allowing them to enhance their scope of practice.

Compared to new registrants, nurses who gain additional registration are older, with an average age of 34.2. The increased age relative to new registrants reflects that additional registrations are typically completed later in a nurse’s career as they are gained after finishing another nursing degree. The variability in the average age of additional RPN registrations is due to the low volume, which range from 23 to 80 total registrations per year.
